Position SummaryUnder the direction of the Manager of Housing Based Support Services, the ICMS Case Manager will be responsible for providing coordinated case management and supportive services for formerly homeless individuals diagnosed with special needs. The Case Manager will deliver services on‑site to tenants who reside at our newly developed multi‑unit permanent supportive housing complex and will help link clients to services including, but not limited to, life‑skills workshops, money management, educational attainment, access to health services, counseling, job training, placement opportunities, and resident activity coordination.
Essential Duties & Responsibilities- Conduct an initial housing intake and needs assessment.
- Assist tenants in determining personal goals and develop an Individual Service Plan (ISP).
- Help clients address pre‑existing conditions or obstacles to facilitate timely housing processes.
- Maintain a minimum of two face‑to‑face contacts with each tenant monthly.
- Conduct quarterly meetings to discuss progress and accomplishments per Individual Stability Plans.
- Act as liaison among Property Manager, property management company, tenant, and primary mental health provider.
- Provide information and instruction on Fair Housing, tenant rights, and property rules.
- Coordinate linkage to community services to support employment, education, money management, and legal/benefits counseling.
- Assist clients in obtaining rental assistance, move‑in/eviction prevention services.
- Address grievances, mediate disputes, and resolve issues with property management.
- Enforce housing rules in collaboration with property management.
- Collaborate with Resident Services Coordinator to develop workshops on housekeeping, money management, parenting, and daily living skills.
- Collect and enter data into DHS CHAMP’s database to document progress and evaluate the program.
- Organize and manage client files.
- Monitor tenants ready to transition to mainstream housing and prepare them accordingly.
- Comply with documentation and reporting requirements per the service contract.
- Submit monthly progress reports tracking residency status and provided services.
- Uphold CRCD’s Mission Statement and 5‑Year Strategic Plan.
- 1–2 years of experience or related work and a BA degree.
- Experience working with youth experiencing homelessness, housing instability, foster care, probation, or mental health systems.
- Reliable, flexible team player capable of working with minimal supervision, good work ethic, and boundary setting.
- Reliable transportation: valid driver’s license and car insurance as required by law.
- Proficient in Microsoft Office suite.
$50,000.00 – $55,000.00 annually
