PIH Health is currently seeking adult and student volunteers. We have 3 categories:
High School (at least 16 years old, sophomore),
College (sophomore), and Adults (not in school). When applying, please indicate your category.
The Patient Care Liaison volunteer assists Care Management Coordinators with scheduling follow-up appointments for patients preparing for discharge. The purpose of this role is to support staff in reducing readmission rates for newly discharged patients, a significant cost savings to the organization. Early follow-up appointments identify issues early, providing effective intervention, patient and family education, and treatment modifications, reducing 30‑day readmission risk.
Connecting the patient and physician improves transition of care.
The volunteer will round with patients to inquire about availability for an appointment, schedule the follow-up appointment, and provide resources regarding insurance and other information.
