Description

Under the direction of the Program Manager, the Time Limited Subsidy Case Manager (formerly Rapid Rehousing Case Manager) provides services to homeless transition-age youth (18-24 years old). The Case Manager assesses needs and connects youth to housing appropriate for their needs, including crisis/bridge, housing, permanent supportive housing, shared, or general affordable housing. The Case Manager assists youth in managing their lives and achieving goals as established in their Individual Action Plan.

The role includes providing each youth on the caseload with referrals for services that include mental/physical health provisions, mainstream benefits, education, employment, and vocational training.

  • Conduct initial intake, needs assessment, and collect the Next Step screening tool to enroll clients in the Time Limited Subsidy Program
  • Carry a caseload of a maximum of 20–25 youth
  • Assist clients in determining personal goals and plans to achieve such goals through the development of the Individual Action Plan (IAP), including the frequency and duration of identified support services
  • Conduct face-to-face contact with each client on a regular basis: once a week for unhoused clients and a minimum of one home visit once housing is secured
  • Partner with partners to outreach to homeless youth in South Los Angeles
  • Establish and cultivate relationships with landlords and other housing providers to build a portfolio of housing options for clients
  • Coordinate services in the following categories: (a) employment training and education; (b) money management counseling; (c) mental and/or medical health services; and (d) legal and benefits counseling
  • Participate in coordinated case conferencing meetings with service providers
  • Develop and facilitate workshops on topics including money management, life skills (including transportation, self-advocacy/community accessibility), achieving independence, and other topics as needed
  • Data collection/entry into HMIS and Salesforce database to assemble reports and evaluate program outcomes
  • Maintain client case files for accuracy of all eligibility-required documents as per CRCD case filing protocols and procedures
  • Compile and submit monthly service reports to supervisor that track clients’ status and the supportive services provided
  • Attend all required LAHSA meetings, relevant community meetings, and staff development trainings as assigned
  • Act as a liaison among the Project Manager, property management company, and primary mental health provider to communicate concerns that could jeopardize the client’s housing
  • Coordinate linkage to services with public social service agencies and community providers to connect clients with employment and education/training
  • Deliver services through CRCD’s Client Centered Approach
  • Uphold CRCD’s Mission Statement and 5 Year Strategic Plan

Requirements

  • 1–2 years’ experience or related work and a bachelor’s degree; or an associate’s degree and at least 2 years’ experience providing housing-based support services to young people facing multiple barriers to housing stability
  • Experience working with youth experiencing homelessness and housing instability, and/or experience in foster care, probation, mental health systems
  • Reliable, flexible team player who works well with minimal supervision, has a good work ethic, and can set and maintain personal boundaries
  • Reliable transportation: valid driver’s license and car insurance as required by law, as appropriate; proficient in Microsoft Office suite
  • Knowledge of South Los Angeles community and social services resources is a plus
  • Background in mental health services, trauma-informed care, and/or harm reduction is a plus
  • Working knowledge of data tracking systems and tools
  • Strong organizational skills to manage a large caseload, data, and reporting duties
  • Aptitude in decision-making and problem-solving
  • Reliable transportation, valid driver’s license, and car insurance as required by law

Salary

$50,000-$55,000 annually
