Community Services Care Coordinator – Overview

Join Children’s Institute as a Community Services Care Coordinator. Support children and families exposed to violence through care packages, screening, referrals, outreach, and community engagement.

• Engage community members and families with empathy following a violence incident.
• Identify client needs and provide referrals; coordinate services with other providers.
• Assist clinicians in service linkage, coaching, and skill‑building.
• Maintain effective communications with stakeholders.
• Establish rapport and work cooperatively with children, families, CII, and community resources.
• Ensure accurate and timely completion of outcomes and reports.
• Maintain knowledge of service standards, policies, contracts, licensing, and governing bodies.
• Advocate on behalf of clients with agencies and government programs.
• Participate in initial intake interviews and complete required paperwork.
• Set up and maintain files and documentation, ensuring compliance with policies.

• Conduct home visits to determine client needs; provide training, guidance, and support.
• Participate in multidisciplinary teams to develop and implement service plans.
• Aid in facilitation of group sessions.
• Perform other duties and special projects as assigned.
