The People Concern in Los Angeles is seeking a Case Manager for the C3 Skid Row program. This onsite role will engage highly vulnerable homeless individuals, link them to housing and services, and provide aftercare and follow-up support as they transition to housing.
You will work with the C3 team, maintain records, and advocate for participants while coordinating care with partner agencies in a high-tolerance Harm Reduction setting.
