Population Health Nurse: Chronic Care & Prevention

Savoy Medical is seeking a Population Health Nurse to coordinate care for patients with chronic disease and promote a shared goal model across care teams in Louisiana. You will partner with patients, families, nurses, physicians and other providers to optimize transitions and preventive visits.
