Job SummaryThe Company is currently hiring a Dialysis NP/PA to provide on-site care to long term care residents and outpatient residents in a dialysis center. This is a full-time position ideal for a provider who values meaningful patient relationships and a collaborative care environment.
Responsibilities- Provide primary care services to dialysis patients in skilled nursing and long-term care facilities as well as outpatient dialysis patients
- Perform physical exams and medical history assessments
- Diagnose and treat chronic and acute health conditions
- Order and interpret diagnostic tests and lab results
- Develop and adjust individualized care plans
- Prescribe medications and monitor treatment effectiveness
- Collaborate with facility staff and interdisciplinary teams
- Collaborate with dialysis center medical director for improvement in patient care and quality metrics
- Interact with skilled nursing and long term care physicians and non-physician practitioner to assure good coordination of care and reduction of avoidable hospitalizations
- Ensure a safe, respectful, and supportive environment for patients
- Clinical management

Perform comprehensive assessments, diagnose conditions, and adjust dialysis treatment plans and medications in collaboration with a nephrologist. - Patient monitoring: Continually oversee patients' health during and after dialysis, interpret lab results, and respond to any complications.
- Interdisciplinary coordination: Lead and collaborate with the facility's nursing staff, dialysis technicians, and other healthcare providers to ensure cohesive patient care.
- Patient and family education: Educate patients and their families on kidney disease, the dialysis process, and critical lifestyle modifications like diet and fluid intake.
- Administrative documentation: Maintain precise and timely patient records, manage medication orders, and participate in quality improvement initiatives.
- Patient advocacy: Serve as the primary point of contact for patients and families, providing clinical and emotional support while coordinating care with specialists.
- Active and unrestricted Nurse Practitioner, Physician Assistant license
- State-specific license in good standing in Louisiana
- Minimum 2 years of experience as a dialysis NP/PA
- Willingness to work with dialysis patients in skilled nursing and long term care facilities
- Strong communication, clinical, and critical thinking skills
- Passion for elder care and person-centered healthcare
- Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
