Field Care Management Behavioral Health

Base pay range

$65,000.00/yr - $88,600.00/yr

The Field Care Manager, Behavioral Health 2 assesses and evaluates members' needs to achieve and maintain optimal wellness. The role involves interpreting and determining appropriate courses of action and guiding members/families to suitable resources.

• Performs telephonic and face-to-face assessments and evaluations of member needs.
• Ensures members progress toward desired outcomes through continuous monitoring.
• Creates member care plans aligned with organizational strategy.
• Collaborates with providers and community services to promote quality and cost‑effective outcomes.
• Ensures delivery of services across Behavioral Health, Physical Health, Social Determinant of Health, and value‑added benefits.
• Coordinates across the transdisciplinary care team and transitions of care.
• Submits incident reports.

Field Care Manager, Behavioral Health must meet one of the following:

• Active, unrestricted Louisiana Licensed Masters Clinical Social Worker (LCSW)
• Louisiana Licensed Professional Counselor (LPC)
• Louisiana Licensed Marriage Family Therapist (LMFT)
• Louisiana Licensed Addiction Counselor (LAC)
• Active, unrestricted Louisiana Registered Nurse (RN) with at least three years of behavioral health experience

Must meet all of the following:

• 2+ years of experience as a behavioral health professional
• Experience with behavioral change, health promotion, coaching, and/or wellness
• Proficiency with Microsoft Office and shared systems
• Must reside and work in Louisiana

• Telephonic patient support experience
• Community mental health or crisis response experience
• Experience working with children and adults
• Bilingual (English/Spanish) proficiency

• Minimum 75% travel within Louisiana territory
• Valid driver's license and reliable transportation
• Participation in Humana’s Driver Safety program with required insurance limits

• Minimum download speed of 25 Mbps and upload speed of 10 Mbps
• Connection may be wired, wireless, or DSL; satellite, cellular, or microwave only with leadership approval
• Employees in California, Illinois, Montana, or South Dakota receive bi‑weekly internet expense payment
• Company provides telephone equipment as needed
• Dedicated space without interruptions to protect PHI/HIPAA information

• Must be separated from agencies per Section 1121 of the Louisiana Code of Governmental Ethics for at least two years if previously employed by LDH.
