Social Worker

Join LCMC Health as a Social Worker, a critical member of the multidisciplinary care team advocating for patients with complex medical and psychosocial needs. As a Social Worker you will coordinate care, identify barriers, and provide counseling and support throughout the entire care journey.

• Conduct psychosocial assessments for patients and families, reviewing medical records and collaborating with interdisciplinary team members.
• Identify barriers to care, develop and implement intervention plans, and document findings in the electronic medical record.
• Provide crisis intervention and supportive counseling for individuals experiencing temporary or situational problems.
• Assess suspected elder, child, sexual, or domestic abuse and comply with mandatory reporting requirements.
• Refer patients/families to community agencies for financial assistance, palliative or hospice services, and other resources.
• Facilitate family meetings, coordinate care plans, and maintain knowledge of payer requirements for post‑hospital services.

• Master’s degree and licensed MSW through the Louisiana State Board of Social Work Examiners.
• Experience providing social work services in a hospital or acute care setting.
• Knowledge of legal rights, advance directives, and patient rights.
• Strong assessment, communication, and crisis intervention skills.
