Clinical Pharmacist – Infectious Disease

Join Ochsner Health to apply for the Clinical Pharmacist – Infectious Disease role. You will manage medication therapy for assigned patients, monitor appropriateness based on institutional criteria, perform medication reconciliation, patient education, and contribute to education of Pharmacy and Medical Residents. You will also provide expert content for order sets, benefit design, formularies, and related documents.

• Medication management and reconciliation for assigned patients.
• Monitor and evaluate medication therapy, including pharmacokinetic consultation and laboratory data analysis.
• Educate staff, residents, students, patients, and health care providers.
• Mentor residents, students, and new staff to meet departmental goals.
• Assist with development of pathways, best practices, and guidelines for the assigned population.
• Adapt communication style to meet the needs of diverse patient populations.

• Bachelor of Science in Pharmacy (Doctor of Pharmacy preferred).
• PGY1 Pharmacy Practice Residency required; PGY2 Specialty Pharmacy Residency preferred.
• Current Pharmacy license in the state of practice.
• Strong leadership, teaching, and communication skills.
• Proficiency with Windows-style applications and keyboard data entry.
