Infu Care Rx seeks an experienced Care Transition Coordinator to join the New Orleans area team. The role focuses on coordinating safe hospital-to-home infusion transitions, educating patients, and building relationships with hospital staff and partners to ensure timely, quality infusion care at home.
The ideal candidate has sales and care coordination experience, is licensed (RN/LPN) or a registered dietitian, and can travel to hospitals and clinics as needed.
