Overview
Make a meaningful impact every day as a Center Well Home Health nurse. You’ll provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes and work closely with physicians and clinicians to develop and manage care plans that support recovery.
ResponsibilitiesAs a Home Health Registered Nurse, You Will
- Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, providing necessary follow‑up as directed by the Clinical Manager.
- Consult with physicians to develop the initial plan of treatment based on orders and assessment; provide hands‑on care, manage and evaluate the care plan, and teach the patient under the Clinical Manager’s supervision.
- Coordinate appropriate care among allied health personnel (Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
- Report patient care, condition and progress to the patient’s physician and Clinical Manager continuously.
- Implement the patient care plan with patient and family to achieve optimal resolution of needs and problems.
- Coordinate, oversee and supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers; provide written care instructions and monitor documentation completion as part of supervisory responsibility.
- Discharge patients after consultation with the physician and Clinical Manager, preparing and completing required clinical documentation.
- Prepare comprehensive medical documentation for all patients, including case conferences, medication changes, re‑certifications, progress updates and care plan changes; provide visit and shift reports, summaries and collaborate with other health disciplines.
- Ensure optimum patient care at all times.
Required Experience / Skills
- Diploma, Associate or Bachelor Degree in Nursing.
- Minimum of one year nursing experience preferred; strong background in medical‑surgical, ICU, ER or acute care.
- Home Health experience is a plus.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance and reliable transportation.
- $45.00 – $63.00 per visit/unit.
- $70,500 – $96,900 per year base pay.
