As a Registered Nurse Home Health, you’ll contribute to our success by:
- Ensuring all activities performed align with the vision of Elara Caring’s board and leadership.
- Maintaining patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
- Assessing assigned caseload of home‑health care patients and families to identify physical, psychosocial, and environmental needs.
- Assuming primary responsibility for caseload: assessing, planning, coordinating, implementing, and evaluating the plan of treatment.
- Assessing patient needs and obtaining data on physical, psychological, social, and spiritual factors that may influence health status, then incorporating that data into the plan of care.
- Making initial evaluation and re‑evaluating the patient’s nursing needs during each visit, and making follow‑up visits according to patient, family, and facility needs.
- Communicating significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved in patient care.
- Revising the plan of care in response to identified patient care issues and notifying the Clinical Supervisor, physician, facility, and other team members.
- Current, unrestricted state RN license.
- 1+ year experience in a clinical care setting as a nurse.
- Able and willing to travel within branch/office coverage area.
- Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Must be able and willing to travel 50%.
- Associate’s Degree or Bachelor’s Degree in Nursing is preferred.
- Previous Home Health or Hospice Experience preferred.
- Previous experience with Home Care Home Base preferred.
You will report to the Clinical Manager or Clinical Supervisor.
