Position: Social Worker (MA/MSW) In Patient

Responsibilities

Centennial Peaks Hospital has been providing psychiatric services to the Denver‑metro area for more than 30 years. Located in beautiful Louisville, CO, our 104‑bed facility offers a peaceful setting for those seeking treatment for mental illness, including chemical dependency. Centennial Peaks offers unique and individualized programming for adolescents and adults that sets us apart from many other treatment facilities. Our dynamic leadership team includes several Licensed Social Workers and a seasoned nursing department.

The hospital provides Electroconvulsive Therapy, Intensive Outpatient, and Partial Hospitalization Programs in addition to inpatient services.

Master's degree in Social Work, Clinical Counseling, Psychology, or Social Services related field REQUIRED

Our Social Workers provide clinical social work services on each of our five units (general adult mental health, adolescent, two intensive treatment units, and a detox/dual diagnosis unit). Working from a trauma‑informed, strengths‑based perspective, they collaborate with patients and their support systems to set them up for success in the hospital and after discharge. Responsibilities include psychosocial assessments, daily process group facilitation, participation in daily treatment planning, processing mental health certifications, crisis intervention, and discharge planning/case management with referral sources and other departments.

Social Workers are a vital part of the treatment team, providing a balanced and patient‑centered approach to care. This is a dynamic, fast‑paced position offering opportunities for growth and development. An ideal candidate will be self‑motivated, organized, able to multitask, and comfortable with crisis situations.

Job Duties
  • Provide patients and families with assessment, interventions, support, advocacy, and case management as a primary provider and/or as ancillary support to the primary medical provider.
  • Practice as a member of a multi‑disciplinary team, providing clinical intervention and support to patients and families.
  • Assess, develop treatment plans, and intervene to address needs identified in the assessment and by the team.
  • Utilize knowledge of culture, age, and developmental stages to develop treatment plans and provide interventions, either as an independent practitioner or as a member of a clinical team.

This is a Tuesday‑Saturday position.

Supervision toward licensure available.
