Nurse Case Manager | Acute Services

A Registered Nurse Case Manager for our Acute Services, you will be focused on Urgent Psych providers and Withdraw Management, and cover for our MAT team as needed. This is an exciting opportunity to address human needs in the community and useyour therapeutic nursing skills to support those struggling withsubstance use and mental illnesses.

You willplayanimportant role, providing nursing services together with an interdisciplinary team of Acute Care Services. Our RN Case Manager supports clients with Urgent Behavioral Health Interventions, Withdraw Management support and, as needed, support behavioral health interventions to engage positively in their communities. As our RN Case Manager for Acute Care Services (ACS), you will work with our Urgent Care Psych Outpatient team. You will support complex care for patients in our locations and services that are part of the ACS programs including, Withdrawal Management and various clinics throughout CFHW, and support MAT services as needed.

You will provide behavioral health care nursing assessment and case management, medication management including monitoring, administration of medication ordering and managing storage of medications. Additionally, you will engage in client advocacy, creative problem solving, and contributions to the plan of care to assist clients to reach healthy outcomes.

• Are you passionate about behavioral health nursing; supporting individuals, vulnerable populations, as they engage urgent psych services, address withdraw management needs, address substance use issues, recovery programs and re-engage as vital participants in community?
• Do you have experience with medication management, administration including medication monitoring, medication training?
• Does the idea of working with adults by engaging them in their recovery and providing health education and services appeal to your professional aspirations to provide the caring of a nurse to those in our communities who are in need?

• Weekday night differential:
  Monday through Thursday 8:00 pm – 7:59 am of $3/hr;
• Weekend day differential:
  Saturday and Sunday 8:00 am – 7:59 pm of $2.50/hr;
• Weekend night differential:
  Friday 8:00 pm – Saturday 7:59 pm, Saturday 8:00 pm – Sunday 7:59 am, Sunday 8:00 pm – Monday 7:59 am of $3.25/hr

• Unrestricted licensure as a Registered Nurse in Colorado or the ability to be licensed in Colorado at time of hire
• One year nursing experience as
  • Bachelor of Science in Nursing (BSN)
    -OR-
  • Associate Degree in Nursing (ADN) with community mental health experience
• Valid BLS (Basic Life Support) certificate at time of hire
