As an independent lease holder in Sam’s Clubs, you will lead patient care, managing eye and vision services while enjoying full autonomy over your practice.

• Establish your days of practice
• Set your hours of operation
• Control your patient schedule
• Determine your fee schedule
• Keep 100% of exam/professional fees

• Equipment/instrumentation
• Administrative and/or Opt-Tech support services (if state allows)

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) in Ophthalmology from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W‑9
