About the job:

The CNA is responsible for providing and assisting participants with activities of daily living and restorative/supportive care necessary to ensure the physical and health safety of the home environment. The role involves monitoring and reporting the health status and ensuring a safe environment for the PACE participants of Senior Community Care of State (SCC). Work shifts are assigned in the SCC Day Center and in participant homes, providing care, services, and support for a safe and effective day program.

Pay: $20-$24 based on experience.

Required Qualifications:
  • Must have an active and up-to-date certification in CPR.
  • Minimum of one‑year experience working with the frail or elderly population.
  • High School diploma or equivalent required.
  • Medical clearance for communicable diseases and up‑to‑date immunizations before having direct participant contact.
  • Valid driver’s license, proof of insurance and means of transportation; travel to participant homes is required.
Essentials:
  • Consults each day with the Home Care Coordinator (or designee) for updates on participant status and care plans.
  • Assists participants with personal hygiene (skin, mouth, hair care, bathing).
  • Assists participants in and out of bed and with ambulation.
  • Helps with prescribed exercise taught by appropriate personnel.
  • Prepares meals, including therapeutic diets, and assists participants with eating.
  • Assists participants with bathroom needs (commodes, bedpans, urinals).
  • Administers medications normally self‑administered.
  • Performs other specific duties such as vitals, changing colostomy bags, and non‑sterile dressing/hygiene care.
  • Provides directed household and homemaker services.
  • At the Day Center, assists with activities of daily living, meal supervision, cleanliness, laundry, and help with loading/unloading participants from vehicles.
