Status: Full-Time, Hourly, Non-Exempt

Program: Housing Stabilization Services

Reports To: Program Supervisor

Pay Range

$37,000.00/yr - $38,000.00/yr

Job Summary and Qualifications

The Case Manager’s primary responsibility is to provide case management services to families who are residing in the Volunteers of America’s Housing programs. This position develops and monitors a service plan in conjunction with each assigned client to empower them, increase their self-sufficiency, and reduce the likelihood of recurring homelessness. The Case Manager will be responsible for assisting the individual/family with obtaining necessary access to medical, social, and educational services, building relationships with Landlords, communicating with property owners, conducting housing inspections, disseminating information about the program, and marketing the program to the community.

Requirements
  • Bachelor's degree in social services preferred.
  • 1 – 2 years’ experience, knowledge of crisis intervention, trauma-informed care, and familiarity with available community resources are essential.
  • Person with flexibility to work as needed to accommodate the schedules of residential homeless programs.
  • Good communication skills, a valid driver’s license, reliable transportation, and ability to attend meetings as needed.
Responsibilities
  • Knowledge of community resources for homeless families.
  • Develop specific housing opportunities for families in the residence.
  • Develop cooperative arrangements with government agencies involved in providing housing for homeless families.
  • Meet regularly with residents and assist them in applying for and following through with housing opportunities.
  • Maintain all records and reports required by the facility and oversight agencies related to the permanent housing goal.
  • Maintain an individual file on all clients, including housing history and notice of acquired community resources.
  • Quarterly reporting.
  • Performance quality improvement (PQI) duties as assigned by supervision & PQI committee.
  • Assist with the facilitation of life skills as requested.
  • Other duties as assigned.
