Job DescriptionThe incumbent in this position through a strength-based approach develops a street and community presence in which to provide education and awareness to youth (12-17), young adults (18-24), and community members and organizations. The incumbent will engage, assess, and intervene with youth and young adults (12-24), providing crisis based case management.
Qualifications- Outgoing personality with strong communication skills
- Available to work evenings and weekends
- Master’s degree in Social Work, can be substituted with Bachelor’s Degree in a related field with three years of experience with homeless populations.
- Minimum of 2 years of experience counseling and or case managing youth at risk or homeless adults; prefer recent experience in crisis intervention.
- Strong multi-tasking skills and time management
- Ability to work cooperatively with different types of personalities
- Well-established writing ability and inter-personal communication skills
- Great attention to detail
- Ability to work independently and without close supervision successfully; self motivated
- Strong computer skills (Microsoft Office) and knowledge of web based applications
- Enthusiastic and energetic team player
- Awareness of and passion for young adults and family issues
- Champions the program’s mission and core values and challenges young people to accept and demonstrate core values of honesty, respect, responsibility and caring.
- Establishes a community street presence in which to provide street based education and support.
- To provide immediate street-based crisis intervention, case management, and referral services to youth and young adults (12 – 24).
- Ensures proper documentation and protection of confidential information about youth, services provided to youth, and contacts with other agency’s personnel about care of the youth.
- Manages emergency situations on a 24-hour on call basis.
- To work with our Basic Center, community TLP staff, and adult homeless coalition to access needed services.
- Provide liaison, advocacy, and referral services with appropriate mental health centers, courts, physicians, school systems, CHFS/DCBS, and other community agencies.
- Maintains consistent and timely records.
- Coordinate and/or provide emergency transportation of young adults.
- Collaborate with community linkages to assist primarily the young adult and their families with appropriate community services to meet their unique needs.
- Engage and educate the community, schools, teachers, school personnel, and student about Safe Place Services through presentations and community events.
- Maintains shared case management offices and rooms in a clean and orderly condition.
- Assures safety and security of youth in cooperation with volunteers and other staff.
- Outdoor work, staff will be exposed to elements of weather, walking on uneven terrain and have contact with homeless populations.
- Walking, climbing, stooping, standing, kneeling, pushing, sitting, crouching, pulling, lifting and carrying up to 35 lbs.
