Description:

The RN implements professional nursing practice centered on the patient as a whole person.

The focus is on the creation of a caring environment centered on healing and wellness.

Nursing practices are based on evidence, current research, and established policies and procedures.

Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity.

Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines.

The role includes coordinating, ensuring, and providing direct patient care activities.

Responsibilities include the provision of nursing rounds, assessments, and treatments.

The RN facilitates admission, transfer, and discharge activities.

Patient and family-centered education, care coordination, and individualized treatment planning are key duties.

The RN performs and documents comprehensive assessments in accordance with scope of practice and policies.

Patient culture, age, and developmental levels are considered during assessments.

The RN identifies patient risk factors and presenting complaints.

Health information is obtained from family or others as necessary.

Objective and subjective data are collected accurately.

The RN verifies initial admission assessments obtained by other team members.

A problem list is developed, prioritizing patient needs based on input from the team.

Actions and interventions relevant to the patient’s culture and age are designed.

The RN determines desired outcomes in coordination with patients and families.

Patient care therapies and procedures are fully implemented within established policies and guidelines.

Procedures, treatments, and processes are explained to patients and families.

The RN initiates actions to alleviate patient stress and discomfort.

Changes in patient condition are recognized, responded to, and communicated appropriately.

Ongoing assessments and responses to procedures are documented according to hospital policy and standards.

Tasks are delegated appropriately, and team members are overseen in patient care activities.

The accuracy and completeness of delegated patient care are monitored by the RN.Educational needs of patients related to prioritized problems are identified and addressed.

The discharge process is facilitated by executing orders in a timely manner and providing discharge teaching.

Input is provided into patient/nurse assignments, considering staffing, activity, and acuity.

Active participation in unit-based activities supporting organizational values and priorities is demonstrated.

A positive group dynamic and healing environment are promoted through words and actions.

Nursing excellence is assured in accordance with accepted standards and evidence-based practice.

Participation in quality improvement activities and core measures is required.

A holistic perspective regarding patient care is maintained.
