Overview
Physical Therapist at Charter Healthcare provides skilled service to the patient with the goal of maintaining the patient highest level of physical function.

Qualifications- Credentials: Valid license as a Physical Therapist by the Physical Therapy Examining Committee of the State Board of Medical Quality Assurance or valid compact Current Basic Life Support (BLS) license.
- Education: Graduated after successful completion of a physical therapist education program approved by CAPTE or successor organizations; or an education program outside the United States determined to be substantially equivalent by a credentials evaluation organization approved by the American Physical Therapy Association or as identified in 8 CFR 212.15(e); passed an examination for physical therapists approved by the State in which services are provided.
- Experience: 1+ years’ experience is desired.
- Core Competencies: Comprehensive knowledge of rehabilitative services in physical and bodily competencies. Ability to plan, organize and make effective decisions. Critical thinking and problem solving. Excellent verbal and written communication and interpersonal skills. Positive communication with all team members.
- Other: Valid driver license and auto insurance.
- Provide treatments as ordered by the attending physician or podiatrist.
- Assist the physician in evaluating the level of function.
- Assist in developing and updating the plan of care.
- Supervise and direct Physical Therapy Assistants as required.
- Observe, record, and report information on the patient condition to the attending physician and in the patient medical record.
- Advise, consult, and, when appropriate, instruct the family and other agency personnel in the patient therapy program.
- Teach and supervise other health personnel when appropriate.
- Evaluate the home environment and make appropriate recommendations for activities of daily living.
- Participate in in-services, team conferences and utilization review as needed.
- Maintains ongoing communication with the agency from start of care and as the patient condition changes, and when the treatment plan changes.
- Submit supplemental orders to be sent to the physician after the initial evaluation and whenever the plan of care is changed.
- Participate in the Quality Assurance and Performance Improvement Plan and activities as scheduled.
- Deliver all required documentation to the offices of the Agency on designated submission days.
- All other duties deemed necessary and appropriate.
