Job SummaryThe Paramedic Case Manager supports TVEMS’s Mobile Integrated Healthcare (MIH) program by delivering patient-centered, community-based care. This role focuses on reducing unnecessary 9‑1‑1 utilization, addressing social determinants of health, and coordinating care to improve long‑term outcomes for individuals and families.
Essential Duties- Assess client physical, mental, and social needs
- Develop and coordinate individualized care plans
- Collaborate with healthcare providers, families, and community resources
- Provide education, counseling, and interventions within scope of practice
- Document care, referrals, home visits, and outcomes
- Follow up with clients after discharge to ensure continued well‑being
- Provide direct out‑of‑hospital patient care as needed
- Support MIH program development and community outreach

Minimum Qualifications- Bachelor’s degree from an accredited college or university
- Current, unrestricted Colorado Paramedic certification or licensure
- Current BLS and ACLS certifications
- Successful completion of TVEMS FTEP program
- Valid Colorado driver’s license with acceptable driving record
- Two (2) years of paramedic, case management, social work, or MIH experience preferred
Physical Requirements
- Work in homes, healthcare facilities, emergency scenes, vehicles, and community settings
- May involve exposure to infectious diseases, hazardous materials, and adverse weather
- Physical duties may include standing, walking, bending, lifting, and assisting with patient movement
- Lifting Requirement – Very Heavy Work:
This position requires the ability to perform very heavy work, including exerting in excess of 150 pounds of force occasionally, 80 pounds of force frequently, and/or more than 35 pounds of force constantly to move equipment and patients. Physical demand requirements exceed those for heavy work.
This position is classified as non‑exempt under the Fair Labor Standards Act (FLSA) and is paid on an hourly basis
. The salary range for this role is $75,511.61 – $ annually
, based on qualifications and experience. Employees in this position are eligible for overtime compensation in accordance with applicable state and federal law.
Final candidates will be required to successfully complete post‑offer, pre‑employment testing and screening in accordance with applicable federal and state law. These requirements may include:
- Job‑related physical ability testing designed to evaluate the ability to perform essential job functions
- Post‑offer medical examination, conducted in compliance with the Americans with Disabilities Act (ADA) and required of all candidates entering this job classification
- Drug and alcohol screening, as permitted by law
- Background investigation, including criminal history and motor vehicle record review
