Job SummaryThe Clinical Care Leader (CCL) is responsible for utilizing the nursing process—assessment, diagnosis, outcome planning, implementation, and evaluation—to provide individualized nursing care in the home setting. The CCL collaborates with inter‑professional colleagues to plan, implement, and evaluate care, documents resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations, and uses technology to enhance quality resident care.
The CCL effectively delegates to and supervises unlicensed personnel, assumes overall nursing accountability for a resident assignment, and serves in leadership roles on department or organizational task forces and committees. The CCL demonstrates competency and practices within the full scope of nursing expertise and uses appropriate population‑specific standards as designated for the specific care area or unit.
- Effectively communicate with peers, inter‑professional colleagues, residents, and families to negotiate and resolve conflicts and issues.
- Demonstrate strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations.
- Coordinate care, resident assessment, resident education, triage, and various nursing interventions for residents in a home setting.
- Operate within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and state administrative rules.
- Maintain compliance with the American Nurses Association’s Scope and Standards of Practice and Code of Ethics and associated interpretive statements.
- Bachelor's degree in Nursing strongly preferred (from an accredited program, e.g., CCNE, ACEN, NLN).
- Minimum of two years’ clinical experience preferred.
- Current, unencumbered RN license with the State Board of Nursing, or valid multi‑state licensure via the Nurse Licensure Compact.
- Basic Life Support (BLS) certification required;
Advanced Life Support (ALS) preferred. - Ability to obtain and maintain required department‑specific competencies and certifications.
- Valid driver's license may be required depending on location.
- Experience with Resident Assessment Instrument (RAI) process is preferred but not required.
