Are you a compassionate Registered Nurse who thrives on building relationships and making sure patients receive the right care, at the right time, in the comfort of their own homes? We’re looking for a Home Health Case Manager who’s organized, clinically skilled, and ready to join a supportive team that values autonomy, teamwork, and work-life balance.
What You'll Be Doing- Manage the Care Journey: Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.
- Conduct Comprehensive Assessments: Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team.
- Coordinate Services: Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes.
- Monitor and Evaluate: Track patient progress, update care plans, and communicate with families and caregivers as needed.
- Documentation: Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy.
- Advocate for the Patient: Provide education, resources, and emotional support to patients and their families to promote independence and wellness.
- License: RN (Registered Nurse) with active (state) license.
- Experience: At least 1 year of experience in home health or case management preferred.
- Skills: Strong critical thinking, communication, time management, and the ability to work independently.
- Tech‑Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or Well Sky is a plus).
- Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.
