• Assess patient needs and develop individualized care plans
• Provide skilled nursing care and patient education
• Manage medications, treatments, and chronic disease care
• Coordinate care with physicians and interdisciplinary team members
• Support patients and families through every stage of care and recovery

• Active RN License (PA and/or NJ)
• Strong clinical assessment and patient education skills
• Home Health, Acute Care, Med-Surg, or Community Health experience preferred
• BLS/CPR Certification
• Valid Driver's License and reliable transportation
