Your role and responsibilities

As a Senior Independence and Wellbeing Practitioner, you will play a key role in helping adults achieve the best possible outcomes, maintain their independence, and improve their wellbeing. Working within a supportive multidisciplinary team, you will undertake strengths-based assessments and reviews, ensuring people receive the right support at the right time.

In this rewarding role, you will:

  • Complete Care Act assessments and reviews to identify eligible care and support needs.
  • Develop, arrange and review personalised care and support packages.
  • Apply the principles of the Mental Capacity Act 2005 to support informed decision-making.
  • Respond to safeguarding concerns in line with local policies and procedures.
  • Complete NHS Continuing Healthcare checklists and contribute to the wider assessment process.
  • Undertake telephone assessments, home visits and face-to-face conversations with people and their families.
  • Work closely with colleagues across health, social care and voluntary sector services.
  • Attend and contribute to professional and multi-agency meetings.
  • Carry out lone working within the community where required.

This is primarily a community-based role involving regular travel, home visits and face-to-face assessments. Hybrid working is supported, with administrative work undertaken from Riverside or from home where appropriate.

You will receive a comprehensive induction, including shadowing opportunities and access to training and professional development.
