Position Summary
Provides prescribed nursing care services to terminally ill, injured, convalescent and disabled persons patients in a home setting, nursing facilities, assisted living facilities and/or hospitals. Functioning independently without on-site supervision. Provides education and support for family members and caregivers. Oversee the clinical and social work activity surrounding the care of the patient and family. Ensures continuity of quality care on nights and weekends.

- Demonstrates knowledge and competency in performing baseline nursing skills.
- Maintains clinical competencies according to agency standards.
- Performs accurate assessments, develops and implements a written Plan of Care with the patient, following physician’s orders and according to Agency guidelines.
- Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change.
- Addresses patient needs and problems, nursing intervention and patient response, safety interventions and education and teaching.
- Administers all medications and treatments according to agency policy and procedure, accepted standards of care and agency guidelines.
- Communicates effectively with patients, families, caregivers, and physicians.
- Documents patient status in an accurate, timely and concise manner in electronic medical record, as required by Agency policy.
- Demonstrates organization and leadership abilities by effective use of time and personnel through supervision and teaching.
- Effectively establishes priorities and delegates responsibilities appropriately when supervising the LVN and the CNA.
- Makes visits as scheduled and participates in on‑call rotation as required.
- Maintains a committed and cooperative attitude with staffing, promoting teamwork, effectiveness and efficiency.
- Work the defined evening and weekend shifts, providing continuity in the plan of care.
- Makes visits as required to ensure quality care.
- Performs other duties as assigned.
- Required:
Graduate of an accredited school of nursing program. - Preferred:
Bachelor’s Degree Nursing. - Required:
1 year nursing‑related experience. - Preferred: 1 year experience in hospice.
- Mathematical
Skills:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. - Language
Skills:
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications. - Advanced communications skills (verbal and written).
- Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
- Assessment skills related to patient and family care.
- Ability to respond to common inquiries or complaints, regulatory agencies or members of the business community.
- Time management and organizational skills.
- Cooperative attitude.
- Ability to operate standard office equipment and electronic medical record.
- Must be able to work nights and weekends.
- Must be able to work seven straight days on.
- Ability to work with a variety of healthcare professionals across a wide variety of healthcare settings.
- Extensive local travel in personal vehicle to provide service delivery.
- Exhibits flexibility and adaptability in a dynamic and fluid environment.
- Appropriate competency assessments completed.
- Required upon hire:
Texas Registered Nurse License (Vendor Managed). - Required within 30 days of hire:
National Provider BLS – American Heart Association (Vendor Managed).
