ResponsibilitiesPerforms advanced vocational nursing work. Provides care, treatment, and general welfare of individuals residing at the State Supported Living Center who have complex medical, mental health and/or behavioral health issues. Performs basic and advanced routine and non-routine nursing tasks to promote health. Collects specimens for lab work and performs health screenings. Instructs direct support professionals and other staff on how to provide service and care that supports the individual’s medical needs.
Administers medications and treatments orally, rectally, internally, or by injection, makes rounds and home visits, demonstrates competency in performing complex technical nursing tasks such as tracheostomy, gastrostomy, wound care, breathing treatments. Monitors weight, diet, vital signs and health status. Documents and reports treatments in the medical record accurately. Fosters interdisciplinary collaboration, commutes information, collects data for RN/Primary Care Provider. Leads, monitors, and oversees others in nursing duties.
Trains and instructs new LVN staff. Assists RNs with protocol implementation, quality assurance, infection control, and therapeutic programs. Responds to emergency situations, follows safety and infection control. Performs other duties as assigned.
Additional pay may be earned for work performed on evenings, nights, and/or weekends.
Essential Job Functions- Attends work regularly and may be required to work a specific shift schedule or rotating schedule, extended shift and/or overtime.
- May serve in an on‑call status on a rotating schedule to meet operational needs.
- Administers medications and treatments, performs rounds and home visits as needed.
- Demonstrates competency in complex technical nursing tasks in accordance with regulations.
- Monitors weight, diet, feedings, and vital signs; monitors health status post‑treatment.
- Documents and reports treatments accurately in the medical record.
- Communicates information to team members; collects data for RN/Primary Care Provider.
- Leads and oversees others involved in monitoring individuals before, during, and after treatments.
- Trains new LVN staff and monitors their work.
- Assists RNs with protocols, quality assurance, infection control, therapeutic programs.
- Responds to emergencies, follows safety procedures.
- Participates in committees and quality improvement initiatives.
- Assists with disaster response and/or recovery or Continuity of Operations activation.
Skills and Abilities
- Knowledge of nursing care standards, nursing procedures and techniques and of the principles and processes used in providing appropriate patient care.
- Knowledge of emergency procedures and emergency equipment (CPR) and ability to assess and care for residents and/or staff members in emergency situations.
- Knowledge of state and federal laws and intermediary care facility rules related to State Supported Living Center.
- Skills in using reasoning to identify problems and consider appropriate resolution of situations.
- Skill in the operation of computers and applicable software.
- Skill in the care and treatment of patients.
