The Outreach Coordinator reports to the Director and is responsible for planning, implementing and managing social services for the Town’s older adults, with emphasis on those who have one or more disabilities and those who are at risk of institutionalization. Work includes assessing individual needs and referring older adults to the appropriate resources for assistance; preparing reports and correspondence; responding to inquiries, requests and complaints;

maintaining and updating files and records; providing follow-up as needed; and performing a variety of administrative and support responsibilities. Successful performance will be enhanced by a candidate who has extensive knowledge of older adults, computer programs and general exposure to public relations as well as a sense of humor and a positive attitude.

Essential Functions

  • Make assessments of older adult residents; identify individuals in need of services; receive information and referrals from outside agencies regarding potential clients and provide follow-up as appropriate.
  • Provide information and application assistance for all benefit programs including but not limited to SNAP, Brown Bag, farmer’s market, RMV transactions, fuel assistance, Medicare and housing.
  • Address the psycho-social/mental health/physical health needs of individuals and families.
  • Conduct home visits to assess needs, coordinate services and monitor service delivery.
  • Provide crisis intervention assistance and referrals to social service agencies, and mental health providers and resources.
  • Complete necessary documentation regarding intakes, services, assessments and referrals and maintain all confidential records and updates as necessary.
  • Provide telephone consultation, education and referrals to Town residents and their families.
  • Develop an information and referral network with various agencies, corresponds with representatives to obtain information and assistance or to refer clients for services.
  • Attend seminars and conferences related to services for older adults to remain abreast of current information in the field.

Performs similar or related work as needed and assigned by the Director or designee to ensure the efficient operation of the Senior Center.

MINIMUM QUALIFICATIONS

Qualified candidates must have a Bachelor’s degree in social services, gerontology, or a related field from an accredited college or university, three (3) years of relevant experience (one year of which must include working with the older adults) or any combination of equivalent education, training and experience; must have knowledge of programs, services and resources for older adults; experience with Microsoft Office products, Outlook and My Senior software preferred, must have a valid driver’s license and be willing to obtain CPR /First Aide certification within 6 months of hire.

Must possess excellent customer service skills with sincere interest in working with older adults and persons with disabilities; advanced computer skills; strong organizational skills; ability to pay attention to detail; maintain strict confidentiality; and be a self-starter. Regular and punctual attendance is a requirement of the position. Additionally, candidate must be able to work Monday – Friday, 8:00 a.m. – 4:00 p.m.

KNOWLEDGE, ABILITY AND SKILL

Knowledge
:
Thorough knowledge of federal and state services and local resources available to older adults; considerable knowledge of gerontology, ability to work with and communicate with older adults and their families; ability to maintain harmonious relationships with others, particularly with older adults; ability to impart vital information for the benefit of on-going services. Experience and knowledge of computers and geriatric services are a must.
