The Home Health Occupational Therapist (OT) assesses and evaluates patient care needs related to functional status, activities of daily living, fine motor skills, home assessments, adaptive equipment, and other occupational therapy needs based on medical conditions.

• Provide services within the scope of practice defined by state laws, following the plan of care, using evidence-based techniques, and collaborating with the healthcare team.
• Evaluate patients' functional status, body systems, and occupational therapy needs. Consult with physicians to develop the therapy plan of care.
• Observe, document, and report patient responses to treatment and any changes in condition to the nurse supervisor and physician.
• Conduct assessments to evaluate functional ability using diagnostic and prognostic tests.

License Requirements

• Current Occupational Therapy license in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation
