Licensed Vocational Nurse; LVN PRN - Home Health

Overview

Licensed Vocational Nurse (LVN) PRN - Home Health role at Doctor's Choice Home Care & Hospice Texas. Provides skilled nursing care to patients in their place of residence under physician orders and the plan of care, under the direction and supervision of the Registered Nurse (RN).

• Administer skilled nursing care per physician orders and plan of care in patients' homes.
• Develop and evaluate the plan of care in partnership with the patient, the patient’s representative (if any), and caregiver(s).
• Participate in patient, caregiver, and family counseling and caregiver education.
• Communicate with physicians involved in the plan of care and other health care practitioners related to the current plan of care.
• Provide services that are ordered by the physician as indicated in the plan of care.
• Contribute to all phases of the QAPI program and participate in ongoing interdisciplinary assessments of patients.
• Be monitored and evaluated by the RN in the provision of service, treatments, patient education, and communication with the RN and data collection regarding the patient s status and health needs as delegated by the RN.
• Adhere to Agency policies in the provision of services.

• Active LVN license in the state(s) of practice required.
• Minimum of one (1) year nursing experience; home health experience preferred.
• CPR certification with American Heart Association or American Red Cross required.
• Ability to exercise initiative and independent judgment.
• Strong communication skills, both oral and written, with excellent interpersonal skills.
• Must demonstrate good customer relations skills and a commitment to providing quality service.
• On-call may be required; 100% travel may be required.
• Valid driver s license, automobile liability insurance, and reliable transportation required.
• Acceptance and ability to demonstrate support for the core values and goals of the Agency.

• High School Diploma or GED required.
• Graduate of a state-approved school of practical (vocational) nursing required.

• Works in patient homes and travels to/from patient homes. Requires considerable physical effort most of the day, including lifting up to 100 pounds with assistance as needed.
• Must possess sight/hearing senses or use appropriate adaptive devices to meet essential duties.
• Must provide evidence of annual TB test and other state-required tests or examinations.
• Be able to tolerate exposure to elements, odors, blood, body fluids, and hazardous materials.
