HOSPICE OFFICE NURSE - LPN

Job Summary:
- Assist in coordinating the transfer of patients from home health services to Palliative or Hospice, resulting in uninterrupted continued medical care.
- Assist in coordinating the transfer of patients residing in a SNF or discharging from a hospital setting to Palliative or Hospice services resulting in uninterrupted continued medical care.
- Provide patient care, to include using a variety of skills and necessary coordination of services.
- Complete appropriate documentation as required by policy and procedure and regulation.
- Assist Clinical Manager and/or Hospice Administrator in day-to-day branch office tasks to ensure patient care documentation is complete and submitted timely.
- Assist Clinical Manager and/or Hospice Administrator in management of patient care supplies.
- Assists in Medicare part D medications management.
- Assists the Hospice Registered Nurse with medication reconciliation and Pharmacist medication recommendations.
Job Requirements:
- Requires current LPN licensure within state of branch location and two years of clinical nursing experience, preferably one year in Home health/Hospice setting. Successfully completed nursing education from an approved school of nursing.
- Must be able to independently organize work and provide effective health care services to hospice patients; ensure appropriate patient care as supervised by the Hospice–Registered Nurse; and document care thoroughly, accurately, and completely on a timely basis.
- Must accept travel and working conditions as assigned.
- Must possess effective communication skills (oral and written), as well as, CPR certification. Minimum of twelve hours of continuing education in a twelve-month period.
