Hospice Registered Nurse at Lower Cape Fear Life Care

• After orientation:
  Monday to Friday, 8:00‑5:00 a.m., with flexibility for patient needs and emergencies.
• Backup on‑call rotation with a dedicated on‑call team.

• Average caseload of 13‑15 patients; can fluctuate to 16‑18 as needed.
• Minimum of 20 visits per week (≈4‑5 visits per day). Visits last 45 min‑1 h, extending for active symptom management when required.
• Flexibility to move visits to accommodate patient needs and regulatory guidelines.

• Provide care in patients’ homes, assisted‑living facilities, and skilled nursing facilities.
• Train caregivers in wound care, medication administration, personal care, and end‑of‑life expectations.
• Travel between patients using personal vehicle; mileage reimbursable.
• Coordinate care with community medical providers, hospice providers, and other disciplines.
• Document care in real‑time at the patient’s home; order medications and supplies online.
• Schedule visits based on patient acuity and team needs, with flexibility for team demands.
• Participate in an Interdisciplinary Team (IDT) meeting every two weeks.

• Graduate of an approved nursing school (AD, Diploma, or BSN).
• Current nursing license in North Carolina or South Carolina.
• Two years of experience, preferably in Home Care, Hospice, or oncology.
