Key Responsibilities:

• Conduct client assessments for tailored care plans

• Provide medication management and wound care

• Collaborate closely with healthcare teams for primary care

• Engage in chronic disease management practices

• Perform dialysis management and rehabilitation programs

Requirements:

• Active Registered Nurse license in Nova Scotia

• Community health experience preferred

• Must have reliable transportation and valid driver's license

• Satisfactory Criminal Records Check required

• Ability to wear Personal Protective Equipment (PPE)
