Job Opportunity:
Community Falls Support Worker

Do you want to make a difference to someone's ability to remain safe and independent in their own home? Do you have experience of working in the community as an HCA, re‑ablement support worker, rehabilitation support worker, therapy assistant, or therapy technician? Do you enjoy working in the Luton community, providing a prevention and rapid inter‑vention service for people who have fallen or are at risk of falls?

The service works shifts from 8 p.m. until 6 p.m., 7 days per week, with weekend work on a rota basis. A typical shift pattern for a full‑time member of staff may include 4 working days over a 7‑day period with 3 rest days.

Due to the nature of the role, it is essential that applicants are able to travel effectively and independently on a daily basis to and from the base, clients’ homes, etc., while transporting equipment. Adjustments can be considered in accordance with the Disability Discrimination Act.

Responsibilities
  • Respond quickly to all referrals for patients who have fallen or are at risk of falling, arranging to visit them as soon as possible.
  • Visit patients in their own home or place of residence.
  • Complete a comprehensive, evidence‑based falls assessment and identify any equipment, additional care or support required to enable the person to remain at home safely.
  • Support the Luton Falls Team in delivering all aspects of the falls strength and balance classes/group programme, including face‑to‑face groups, virtual sessions (both live and pre‑recorded), and home exercise sessions.
  • Provide basic mobility equipment, demonstrate use and educate patients and carers. Make recommendations to reduce the risk of further falls.
  • Refer to other services where appropriate to reduce fall risk and promote independence.
  • Follow up patients to ensure equipment has been delivered and care is in place.
  • Maintain and update patient records in a timely manner using an electronic system, collating and analysing associated data with the Falls Coordinator.
  • Undertake related administrative duties, such as completing notes and onward referrals to Social Services, safeguarding, Community Rehab, Assistive Technology Services, Age Concern, Total Wellbeing, Fire Service, Medequip and other specialist services with the Trust.
  • Plan and prioritise own workload, work autonomously according to service protocols, procedures and pathways, and contribute to the development of efficient operational procedures and local pathways.
  • Organise and run group activities, support the team lead in completing audits, and support the team in promotion activities within Luton through other service partners.
Benefits and Working Conditions

The Trust is rated 'Outstanding' by the Care Quality Commission and has a proven track record of high staff survey outcomes. We offer a supportive and friendly team environment, training and support for staff development, IT literacy, and varied interesting work. Trust flexibility, including job‑share, compressed hours and tailored working patterns, is considered for all roles.
