Role overview: As an Adults Social Worker in the Long Term Planning Team
, you will manage long-term and complex cases, delivering Care Act–compliant assessments, reviews, and support planning. You will work in partnership with individuals, carers, and multidisciplinary teams to promote independence, wellbeing, and safety.

What you will do:

• Assessment & long-term support planning
• Conduct Care Act assessments, reviews, and reassessments for adults with long-term needs
• Develop and review person-centred care and support plans focused on independence and wellbeing
• Manage long-term cases, ensuring support arrangements remain appropriate and effective
• Identify, assess, and respond promptly to adult safeguarding concerns
• Complete comprehensive risk assessments and contribute to safeguarding plans
• Work in line with local safeguarding adults procedures
• Mental capacity & legal frameworks
• Undertake Mental Capacity Assessments and make best-interest decisions
• Apply relevant legislation including the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act
• Support individuals, carers, and families to understand their rights and available options
• Work collaboratively with health professionals, housing services, voluntary organisations, and care providers
• Participate in multidisciplinary meetings, reviews, and case discussions
• Case management & reviews
• Manage a caseload of adults with complex and ongoing needs
• Review commissioned services and support packages for quality and value for money
• Undertake carers’ assessments and reviews where required
• Recording & professional practice
• Maintain accurate, timely, and analytical case records
• Engage in supervision, training, and continuous professional development
• Contribute to service improvement and quality assurance activities

Person specification / requirements:

• Degree in Social Work (BA / MA / DipSW)
• Registered with Social Work England
• Experience with in Adult Social Care
  , particularly long-term or ongoing support teams
• Proven experience undertaking Care Act assessments, reviews, and safeguarding work
• Experience supporting adults with complex needs, including learning disabilities, physical disabilities, mental health needs, or older adults
