Luton Borough Council is seeking an experienced RNLD or RNMH to join our Adult Social Care team at Luton Town Hall. You will assess needs, manage complex cases, and support adults with learning disabilities, physical disabilities, dementia, and other care needs to achieve positive outcomes.
Working within the framework of the Care Act 2014 and Mental Capacity Act 2005, you will deliver person-centred assessments and create support plans that promote independence, wellbeing and choice.
