Job overview

Do youwant to make a difference to someone’s ability to remain safe and independent in their own homes?

Are you educated to NVQ Level 3 or Equivalent and have experience of working in the community as a health care assistant, reablement support worker or in an occupational therapy or physiotherapy setting?

The service works shifts from 8pm until 6pm, 7 days per week with weekend work on a rota basis. The applicant would be expected to work shifts across. A typical shift pattern for a full time member of staff may include 4 working days over a 7-day period with 3 rest days.

We have an exciting opportunity for motivated, well organised and enthusiastic individuals to join our falls team as a Part time Community Falls Support worker here in Luton. We are a small proactive team that work autonomously using your own initiative, have good IT and communication skills. Full support and training will be provided and the Trust is committed to staff development.

The team works with people who have fallen or are at risk of falling in their own homes and aims to prevent unnecessary hospital admissions and reduce the risk of falls.

.

Main duties of the job
  • To work within the Falls Team in providing timely intervention service for people who have had a fall or are at risk of falling.
  • To complete a comprehensive falls assessment and identify any equipment, care or support required to enable the patient to remain at home safely and avoid unnecessary hospital admission.
