Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are seeking highly skilled and motivated ACPs, ANPs & APNPs to join our bank workforce with opportunities within Out Of Hours. We are recruiting for out bank, where we offer the flexibility to fit around you.

Salary: £33.18 per hour

Department: Out of Hours, 24/7 service

Location: Various locations within Merseyside, Sefton & Halton; including Wavertree Head Office.

Shifts: Bank, where you can bid for the shifts you want to work

• Assess, diagnose, plan, implement and evaluate treatment interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
• Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
• Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
• Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
• Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national local protocols and within scope of practice.
• Work with patients to support compliance with and adherence to prescribed treatments.
• Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.

• Within your professional boundaries you will assess, diagnose and provide advice, care and management to patients presenting with undiagnosed or undifferentiated illness via telephone consultations, or face to face assessments for patients requiring home visits.
• Undertake face to face and telephone triage consultations
• Interpret pathology/radiology results/reports
• Prescribe and review medication
• Plan, implement and evaluate an effective management plan for the patient

• You will be an experienced ACP who is able to deliver quality individualised care to our patients with a high level of autonomy underpinned by the four pillars of Clinical practice, leadership and management, education and research.
• Full NMC Registration with no restrictions
• MSc in Advanced Clinical Practice is desirable;…
