An exciting opportunity to undertake the role of Community Mental Health Nurse (Band
6) within the Community Mental Health Team (CMHT), utilising specialist clinical knowledge and experience to provide high-quality assessment, care planning, intervention and review for individuals with complex mental health needs living in the community.

The post holder will work autonomously within professional boundaries, providing specialist nursing interventions, coordinating care, and contributing to multidisciplinary decision making from referral through to discharge. The role involves partnership working with patients, carers, families and a wide range of statutory and voluntary agencies to promote recovery, independence and social inclusion.

The post holder will provide specialist advice and clinical leadership within the multidisciplinary team and contribute to service development, evidence-based practice, and quality improvement. They will also support, supervise and educate junior staff and students.
