What you will do

  • Undertake proportionate, strengths-based assessments of adults’ care and support needs.
  • Develop person-centred care and support plans that promote independence and wellbeing.
  • Respond to changing circumstances through timely reviews and reassessments of care arrangements.
  • Conduct adult safeguarding enquiries in accordance with relevant legislation and procedures.
  • Apply the principles of the Mental Capacity Act 2005 within assessments and care planning.
  • Manage an allocated caseload and maintain accurate, comprehensive case records.
  • Participate in multidisciplinary meetings and work collaboratively with partner agencies and professionals.
  • Support adults to maximise choice, control, independence, and positive outcomes.
  • Work with families, carers, advocates, and community organisations to coordinate effective support.
