Eden Brown Synergy is currently recruiting for a Qualified Social Worker
- Adults to join the Supporting Independence / Community Care Team in Luton.

About the Role

The Adult Social Care Division is committed to promoting the independence, inclusion and well-being of adults with social care needs. The team supports adults and older people to live safe, healthy and fulfilling lives, with choice and control over the support they receive.

Key Requirements
  • Must be a Registered Social Worker (SWE) with current registration.
  • Relevant Social Work qualification.
  • Relevant experience within Adult Social Care.
  • Experience undertaking assessments, support planning and reviews.
  • Experience working with adults with complex needs, including physical disabilities, sensory impairment, dementia or multiple needs.
  • Knowledge and understanding of relevant adult social care legislation, safeguarding and statutory responsibilities.
  • Strong communication, assessment and case management skills.
  • Ability to work effectively with individuals, carers and multi-disciplinary professionals.
  • AMHP and/or BIA qualification would be advantageous where applicable.
Key Responsibilities
  • Receive and coordinate complex referrals and undertake assessments.
  • Undertake support planning and reviews.
  • Promote enablement, reablement and independence.
  • Arrange appropriate equipment, support, personal budgets and person-centred care services.
  • Work with adults with physical disabilities, sensory impairment, dementia and complex needs.
  • Support individuals who may be at risk of harm or neglect, in line with statutory responsibilities.
  • Promote choice, control and personalised care pathways.
  • Work collaboratively with individuals, carers, partner agencies and other professionals.
  • Contribute to improving local services and promoting social inclusion.
  • Undertake AMHP and/or BIA responsibilities where appropriately qualified.
