The position is an advanced practitioner who works as a key member of an integrated Care Team—including a Registered Nurse and a Social Worker—providing compassionate, patient‑focused care primarily to patients with chronic kidney disease (CKD) and end‑stage renal disease (ESRD/ESKD). The goal is to improve quality of life in the home and slow the progression of kidney disease while delivering evidence‑based, timely care that reduces avoidable hospitalizations.
- Conduct comprehensive annual wellness exams, both in patients’ homes and virtually.
- Counsel and educate patients and families about benefits, programs, and lifestyle modifications.
- Document all clinical information—chief complaint, diagnosis, history, exam, medications, allergies, assessment, and plan—accurately and promptly.
- Coordinate care with primary care providers, specialists, and ancillary services.
- Maintain quality‑of‑care standards as defined by the practice.
- Operate remotely in the state of employment, attending periodic trainings or meetings outside that state.
- Deliver evidence‑based care that prioritizes patient satisfaction and reduces avoidable hospitalizations.
- Prescribe medications, order tests, and collaborate with the Monogram physician.
- Measure and report patient quality outcomes daily.
- Facilitate preventive monitoring and scheduling of diagnostic testing.
- Assist patients in enrolling for educational videos and other resources.
- Participate in integrated care team meetings.
- Apply knowledge of disease diagnosis and prevention to develop and implement individualized treatment plans.
- Follow up and evaluate patient status; perform other duties as assigned.
- Bilingual (English/Spanish) preferred.
- Basic Life Support (BLS) certification required.
- Active, unrestricted license as a Registered Nurse and Nurse Practitioner or Physician Assistant.
- Board certification in NP (ANCC/AANP) or PA (NCCPA) required.
- Current, unrestricted DEA certificate.
- Ability to work autonomously without direct supervision.
- Access to reliable transportation, valid driver’s license, and car insurance.
- Proficiency with medical instruments, equipment, and electronic health record systems.
- Strong communication skills in verbal and written form with patients, families, physicians, and community partners.
- Sound understanding of HIPAA, OSHA, and other federal and state regulations.
- Minimum 2 years of direct patient care experience.
- Experience in Managed Care/IPA/Health Plan settings.
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.
