Job Responsibilities
  • Admissions, Discharge and Transfers of residents
  • Medication requisitions with pharmacy and physicians
  • Medication administration
  • Follow infection control, emergencies, and other center policies
  • Collaborate with resident’s interdisciplinary team to provide the best outcomes
  • Identify and report all abnormal changes in residents’ condition to proper personnel per center policy
  • Work under direct supervision in accordance with the state‑specific Nurse Practice Act, facility policies and procedures, and nursing judgment.
  • Deliver nursing care to patients/residents requiring long‑term or rehabilitative care.
  • Collect patient/resident data, make observations, and report pertinent information related to the care of the patient/resident.
  • According to state‑specific regulations, implement the patient/resident plan of care and evaluate the patient/resident response.
  • In accordance with state‑specific regulations, direct and supervise care provided by other nursing personnel in selected situations.
  • Maintain knowledge of necessary documentation requirements.
  • Maintain knowledge of equipment set‑up, maintenance and use, e.g., monitors, infusion devices, drain devices, etc.
  • Maintain confidentiality and patient/resident rights regarding all patient/resident/peronal information.
Qualifications
  • LPN (Licensed Practical Nurse) licensed by the State
  • CPR Certification required
  • Two years prior nursing experience preferred
