Base pay range: $24.00/hr - $36.00/hr

• Travel required to support multiple locations within a 150-mile radius of permanent address
• Mileage reimbursement provided for required travel
• Minimum of 30 hours per month
• One weekend shift per month

• $5/hr premium on all hours worked
• $1.25/hr weekend shift differential
• $1.25/hr evening/night shift differential (6:00 PM – 6:00 AM)

• 3 months experience in Long Term Care required (6 months preferred)
• Minimum of 18 years of age

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse or physician. Demonstrates knowledge of nursing principles and practices within the full scope of nursing expertise. Performs testing, evaluates resident status, administers medications or treatments, documents care, supports interdisciplinary team, counsels families, and follows all applicable policies.

• Graduate from an accredited practical nursing program as a Licensed Practical Nurse. Associates degree in practical nursing is preferred.
• Currently holds an unencumbered Licensed Practical Nurse (LPN) license.
• Minimum of 400 hours of experience as LPN at another facility if required.
• Obtains and maintains required competency certifications and CEU.
