Salary: £24,500.00 per annum (FTE £40,900)
Location: Lymington, Hampshire
Working Pattern: 22.5 hours per week (flexibility required across service needs)
Job DescriptionWe are looking for a compassionate and skilled Community Registered Nurse to join a dedicated Community Nursing Team, delivering high-quality palliative and end-of-life care within patients’ own homes and care settings.
This is a highly rewarding, hands‑on role where you will provide holistic, person‑centred care to patients and their families, while also offering telephone advice and support via a single point of access service.
You will play a key role in supporting both patients and healthcare professionals, acting as a clinical resource to promote best practice in palliative care across the community.
Key Responsibilities- Deliver holistic palliative nursing assessments and hands‑on care in community settings
- Plan, implement, and evaluate person‑centred care plans in partnership with patients and families
- Provide responsive and timely care, including urgent interventions when required
- Offer emotional, psychosocial, and spiritual support to patients and families
- Provide guidance and reassurance via telephone support services
- Promote patient choice, including preferred place of care and end‑of‑life planning
- Act as a clinical resource and educator to health and social care professionals
- Work collaboratively within a multidisciplinary team
- Maintain accurate documentation and contribute to audits and service improvement
- Adapt to the needs of the service with a flexible and proactive approach
- Registered Nurse (NMC – Part 1 or
2) - Recent experience within palliative care, oncology, or end‑of‑life care
- Proven ability to support patients and families through complex and sensitive situations
- Experience working within a community setting (desirable)
- Excellent communication skills, both face‑to‑face and over the phone
- Strong organisational, IT, and problem‑solving skills
- Full UK driving licence
- Compassionate, calm, and professional approach
- Confident and approachable, with strong interpersonal skills
- Ability to manage emotionally challenging situations
- Flexible, adaptable, and team‑focused
