This position is responsible for providing transportation services for Centra PACE/Ground Transport under the direction of the Transport Supervisor(s). Transportation will be provided to PACE participants and patients to & from Centra facilities. The Driver will provide a safe and enriching experience for every individual from door to door. The Driver will possess good communication skills and perform pre‑ and post‑vehicle safety inspections, follow scheduled transports, report and handle vehicle maintenance issues, and communicate all participant/patient needs to the immediate supervisor.

• Providing exceptional door‑to‑door care to all PACE Participants & Centra Patients
• Communicating all participant/patient changes & needs to the direct supervisor
• Performing pre‑ and post‑vehicle safety inspections, assuring vehicle is in proper operational condition and reporting any maintenance issues to the immediate supervisor
• Maintaining cleanliness of interior & exterior of the Centra vehicle
• Following scheduled transports for PACE participants or patients & other potential trips communicated by the direct supervisor
• Maintaining all necessary records/documentation regarding participants/patients & vehicles
• Communicating frequent changes to the direct supervisor
• Performing other duties as assigned

• Entering safely into participant’s homes on a routine basis
• Participating as an extension of the Transport Supervisor as a member of the interdisciplinary team & participating in participant’s care plans
• Meeting standardized competencies for the specific position established by Centra PACE and regulatory agencies
• Delivering participant medications, medical supplies, and other medical necessities

• Valid Virginia driver’s license
• For PACE:
  One year’s experience working with a frail and elderly population exhibiting the skills needed to assist geriatric patients with transportation needs
• Pre‑releasing solo: CPR, First Aid, Stair and Gait training, Wheelchair training, fall prevention training
• Emergency Vehicle Operators Course (completed within six months of hire for PACE employees; completed before release for ground transport)

• Previous pre‑hospital EMS or transport experience
