Job SummaryThe Case Manager provides intensive case management services including continuing care placement, assistance with FMLA, housing, and other community supports.
Key Responsibilities- Maintain active caseload providing intensive case management services throughout the course of a patient’s treatment.
- Work collaboratively with the treatment team to assist clients in determining continuing care placement.
- Assist clients with locating sober living homes or extended care.
- Through the combined use of personal vehicles, public transportation, and agency vehicles, case managers will actively assist clients with conducting activities in the community.
- Assist with reintegration into the community and independent living.
- Link clients with appropriate community resources and assist in implementing self-directed service plans.
- Provide a wide variety of client services such as counseling, referrals and resources, life skills training, employment assistance, and health maintenance.
- Collaborate with outside treatment providers and other partner agencies.
- Participate in meetings with service providers including physicians/psychiatrists to assist clients in achieving their goals.
- Create and maintain client files and assist clients with completing any program documentation.
- Complete all program documentation in an accurate and timely fashion.
- Performs all other duties as assigned.
- Bachelor’s Degree in a human service field.
- Peer Recovery Specialist Certification or CSAC (or under supervision).
- Hands-on experience in social and human services field dealing with diverse populations preferably in a case management role.
- Knowledge of procedures of case management, housing needs, trauma informed care, and harm reduction.
- Ability to negotiate and maintain positive relationships with co-workers and clients.
- Skilled in advocating for clients.
- Direct care position
- 40 hours per week, Monday-Friday 8:30am-5:00pm Monday-Friday, with weekend rotation

The pay range for this role is: 50,000 - 55,000 USD per year (Lynchburg)
