Your Role as an RN Case Manager:

  • Travel within a 40-mile service area radius of the Bedford/Lynchburg office location for patient visits with generous mileage reimbursement paid out bi-weekly.
  • Coordinate and oversee care for hospice patients, ensuring comfort and dignity
  • Develop individualized care plans and collaborate with an interdisciplinary team
  • Provide education and emotional support to patients and families
  • Advocate for patients’ needs and ensure high-quality, compassionate care
  • Provide direct patient care in homes and contracted facilities

What You Bring:

  • Reliable form of transportation
  • Hospice or home health experience preferred, but not required
  • Strong clinical and assessment skills
  • Compassion, empathy, and a heart for serving others
