The Medication History Associate I has the responsibility of obtaining an accurate and complete account of the patient's current medications.

• Collects the medication history for the admitted patient for the Admitting Physician to review and reconcile medications
• Utilizes AllScripts to review history of patient’s home medication and allergies
• Utilizes McKesson to place the patient’s medications and allergies into the EMR accurately
• Works collaboratively with the Admitting Physician, ED Physician, ED Nurses, and Inpatient Nurses to ensure an accurate account of the patient’s current medications
• Researches patient’s current home medications by working collaboratively with retail pharmacies, nursing homes, families, hospitals, or doctor’s office
• May perform other duties as assigned or requested and job specification can be modified or updated at any time

  Education:
  
  High School Diploma / GED
• Preferred Experience:
  
  Pharmacy Tech with at least one (1) year of experience as a Pharmacy Tech in a hospital or retail pharmacy setting. Knowledge of both the generic and trade names of medications
• Required Certifications and Licensures:
  Hold a current, active American Heart Association Basic Life Support (AHA BLS) course completion card.
